Comprehensive assessment of patients and family caregivers

Explaining about the structure of health system, the role of the comprehensive health service center and their caregivers in the care of cancer patients, how to reach health services in the community and use narcotic drugs, how to refer to clinics and hospitals in the second level of prevention, assessment of the palliative performance of the patient, designing care plans.

Visiting the patient’s home by the coordinating nurse

Reassessment of the patient’s physical and psychological symptoms, including pain, fatigue, loss of appetite, nausea, vomiting, shortness of breath, insomnia.

Follow up

Every two weeks, the researcher visited the patient’s home after coordination with the comprehensive health center for re-evaluating the patient, delivering the necessary training, and following up with the condition of the patient and his/her family.
